Easily make a payment
using your credit card or digital wallet.

Just click on the appropriate button below for your type of appointment.  The button will take you to Square where you can safely make a payment using a variety of credit cards such as VISA or MasterCard or selected digital wallets including CashApp.  All transactions are secure and encrypted. Super simple!

You can even opt for Afterpay which divides your payment into four interest-free installments.  We - not you - pay the associated fee for this option.

You will receive an electronic receipt from Square after making a payment.

Some HSA cards do not work with Square.  So, if you have problems using your HSA card, please use another form of payment. 

If you are a new patient, then your payment must arrive with your appointment request and with your new patient paperwork.  If you are a follow-up patient, then your payment must arrive at least 48 hours prior to your appointment and with your follow-up paperwork.

Keep in mind that no refunds are offered.

APPOINTMENTS

We offer three levels of regular appointments based on our time: 2 hr, 90 min, and 1 hr.   These appointments consist of the telehealth portion via Zoom plus our usual 30 min administrative portion related to your appointment.  You can choose the length with the associated fee for your appointment.

You do not need to stay on the Zoom call while we finish up administrative tasks related to your appointment.  Nope!  You are free to resume your daily activities.

NEW CLIENTS

The 2 hr appointment is recommended for most first-time clients with chronic and/or complex issues so that we have adequate time to discuss your health concerns with you in depth.

However, you can choose the 90 min appointment for simpler chronic concerns or the 60 min appointment for much shorter situations.

FOLLOW-UP CLIENTS

The 90 min appointment is best for those with complex health concerns and/or to review lab work or other test results all of which require more time.  Most clients need this 90 min appointment for the first few follow-ups.  Thereafter, the 1 hr appointment can be chosen.

Often family members are scheduled back-to-back for follow-ups.  To make it easier for you, use the button for TWO follow-up visits.

ADMINISTRATIVE WORK

Our philosophy is to keep consultation fees as low as possible. We believe that it is much more fair to keep rates low for everyone, and only charge extra for those who need extra services. Most patients do not need any additional services, but some patients want various forms, paperwork, or services. Dr. Bazylewicz may perform simple administrative tasks for an additional charge on a case-by-case basis. Please see the FAQ page for more information.
